Food Allergy & Celiac Disease Friendly Food Pantry


  • We provide free educational materials from FARE, CSA, GIG and other organizations, and general support during pantry hours, which is especially helpful for those newly diagnosed;

  • We offer assistance in finding medical practitioners and

  • We can offer referrals for support


Listed below are requirements for services set by Food Equality Initiative. There may be additional requirements for services set by the organizations we work with. Please contact the pantry directly to inquire about additional requirements. There are no residency restrictions at either pantry location. 

1. Medical Qualifications:

Client must be diagnosed with one of the following conditions:

  • Celiac disease

  • IgE mediated Food Allergies (including anaphylaxis)

  • Other IgE mediated Food Allergies (including anaphylaxis), Oral allergy syndrome, Atopic Dermatitis (Please Specify):

  • Non-IgE mediated: Food Protein-induced Enterocolitis syndrome (FPIES)

  • Mixed IgE and Non-IgE mediated: GI food allergies (Eosinophilic esophagitis or EoE, EGE, Allergic Proctocolitis etc) (Please Specify)

  • Cell mediated: Allergic contact dermatitis, Heiner syndrome

You will need to bring proof of income to your appointment.  It should include your most recent IRS form 1090, your last 3 paystubs and any other income you receive such as disability or unemployment.

****If your total household income is at or below the numbers above, you qualify for the pantry.  However, we understand that these conditions can be very costly, so if your income is above the listed requirements, but you are still struggling to pay for groceries, please fill out the form below to see if we can still help.


3. Medical Qualifications Form

Please pre-register by filling out the form, and bring the Physician Diagnostic form (links below), completed by your physician to your intake appointment.  Someone from our organization will call you and get you registered. Clients without proper documentation will not be served.  Many doctors will NOT need an office appointment to fill out the above form. Physician groups may fax completed Rx forms to the following number.
